Overview of the job    

The job holder is strategic lead on strengthening and developing continuity of care pathways so those leaving prison can access treatment and support on release, as part of the organisation’s aims on reducing/breaking the cycle of reoffending.

As a strategic role within the Probation Service (PS), post holders are accountable to the Head of Community Integration.

The role will work regionally and be based within a community probation office with the expectation that post holders will travel as and when required, including regular visits into selected prisons.

In line with PS policies and procedures, the job holder must at all times demonstrate a commitment to equality and inclusion and an understanding of their relevance to the work they do.

Summary    

The job holder will be expected to lead on continuity of care pathways working in tandem with relevant local partners and HMPPS colleagues to develop practice related to substance misuse and health-related release planning.

This includes designing and consulting on processes to enable information sharing, improving the use of substance misuse and health-related licence conditions, and understanding of health and substance misuse resettlement pathways.

The post holder must adhere to all policies in respect of the sensitive/confidential nature of the information handled whilst working in this position.

Responsibilities, Activities and Duties    

The job holder will be required to carry out the following responsibilities, activities and duties:

•    Develop effective relationships with key partners including prisoners, people on probation, prisons, treatment providers in custody and community and commissioners to identify opportunities to improve join-up, locally innovate and drive forward national priority workstreams.
•    Lead strategic discussions with partners and stakeholders to ensure a whole system approach to the delivery of health and justice pathways from custody to community setting.
•    Use performance data to drive activity to improve probation and prison practice and inform partnership working. With a key of focus on Substance misuse and continuity of care pathways. This is also to include engagement with prisoners and people on probation to understand how to improve health and substance misuse pathways.
•    Lead joint partnership working with health and substance misuse partners to design a more integrated health and justice partnership, focusing on continuity of care pathways. Where the service exists, drive improvements in service integration between probation and NHS England and Improvement’s Reconnect service.
•    Identify good practice and innovation opportunities to improve substance misuse and health continuity of care pathways. Drive these forward locally and share with national teams.
•    Provide a key interface between national Offender Health teams within HMPPS and regional probation delivery. This includes escalating local concerns and needs and ensuring communication and delivery of agreed
national activity.
•    Increase understanding within Probation about good practice and health and substance misuse pathways to support continuity of care, operating as a specialist point of contact across the region for both Probation and its partners.
•    Drive partnership working with the prison Drug Strategy Leads and PGD leads offering advice and support to manage the transition into the community to improve substance misuse continuity of care.

The job holder must be able to fulfil all spoken aspects of the role with confidence in English or (when specified in Wales) Welsh.

Some roles in probation require employees to have access to additional intelligence systems that require a higher level of vetting.  An example of this, is the multi-agency ‘Visor’ system. The reoffending rate has remained broadly static at around 29% since 2010. Reoffending is costing society approximately £18bn per year and almost half of all prisoners reoffend within 12 months of release. Following Dame Carol Black’s two part Independent Review of Drugs, MOJ and HMPPS committed to; Introduce Health and Justice Partnership Coordinators in every probation region in England by 2024/25. We know that many people on probation have a substance misuse need, but also have mental health problems and experience poorer health than the wider population. Health Justice Coordinators are working across the 12 Probation Regions to improve a range of health outcomes for people on probation. The role involves building and improving relationships with a range of stakeholders in the community and in custody, to ensure there are clear pathways into treatment and other health provisions – for all people leaving prison or sentenced to community orders. The Health and Justice Coordinator will work strategically to support the vision of the Yorkshire and Humber Health Justice Team on a number of core priorities; ensuring people access appropriate health services on release from prison, also known as ‘continuity of care’. Increase the number of people dependent on alcohol or drugs to receive a Community Sentence Treatment Requirement (CSTR) as part of a robust community disposal, also therefore diverting people from custody. Support the implementation of the new primary Mental Health Treatment Requirement (MHTR) by leading strategic meetings of Probation and partner agencies to ensure clear pathways for assessment and management of all CSTRs by sentence management teams. Contribute to our work as a Health and Justice Team to better understand the learning from those who sadly die under supervision and the cause is substance related or self inflicted. Presenting learning to staff to increase confidence in working with these vulnerable groups of people, with a focus on suicide prevention and quality delivery of substance related interventions. Evidence shows that people on probation are in poorer health than others in the wider community. The Health and Justice Team are wanting to raise awareness of the Health Inequalities people on probation experience, relating to the Core20plus5 work by health partners. Multi agency working to raise staff knowledge in this area is a key strategic focus currently for the team. The regional Health and Justice Team have been in post two years and it is a unique and rewarding role, focusing on supporting operational delivery. There is a high degree of partnership working both with local partners but also with HMPPS and the pivotal role of linking prisons to communities. The role will include links into national teams working on substance misuse and improving health outcomes, supporting the design and implementation of policies, as they are created and then shared to the region. The area of responsibility for the post will initially be: North Yorkshire and Humberside. The role will also include work across the Yorkshire and Humber Region and some cross regional elements. The postholder will be based at a relevant Probation Office and required to travel to Prisons within Yorkshire and the Humber.

Working Arrangements & Further Information

The MoJ offers Hybrid Working arrangements where business need allows. This is an informal, non-contractual form of flexible working that blends working from your base location, different MoJ sites and / or from home (please be aware that this role can only be worked in the UK and not overseas). Some roles will not be suitable for Hybrid Working. Similarly, Hybrid Working will not suit everyone’s circumstances. Arrangements will be discussed and agreed with the successful candidate(s) and subject to regular review.  

For nationally advertised roles, the successful candidate(s) will be appointed to a MoJ office location, which may include their nearest Justice Collaboration Centre or Justice Satellite Office. This will be discussed and agreed on the completion of pre-employment checks.

Some of MoJ’s terms and conditions of service are changing as part of Civil Service reform. The changes will apply to staff joining MoJ who are new to the Civil Service. Staff joining MoJ from other civil service employers will transfer onto the new MoJ terms if they are already on 'modernised' terms in their current post or onto 'unmodernised' MoJ terms if they are on 'unmodernised' terms at their current post. Standard full time working hours are 37 hours per week excluding breaks which are unpaid. HMPPS welcomes part-time, flexible and job-sharing working patterns, where they meet the demands of the role and business needs.
